This installation guide applies to FORM Lighting LED Puddle Lights for the following vehicles:
- 2019-2026 Chevrolet Silverado 1500

Installation Instructions
Step 1 — Remove the factory puddle light
Locate the factory puddle light on the underside of the side mirror. Use a small pick tool to depress the locking tab, then carefully pry the light downward and remove it from the mirror housing.
Step 2 — Disconnect the factory puddle light
Pull the factory connector straight off the terminals on the back of the factory puddle light. Set the factory light aside. It will not be reused.
Step 3 — Connect the FORM Lighting puddle light
Insert the terminals on the new FORM Lighting puddle light into the factory connector. Make sure the terminals are fully seated in the connector.
Step 4 — Install the new puddle light
Insert the new puddle light into the factory opening and press it into place until the locking tab engages. Repeat Steps 1 through 4 on the opposite side of the vehicle.
Step 5 — Test the puddle lights
Test the puddle lights by covering the light sensor on the dashboard or moving the vehicle into a dimly lit area. Use the key fob to unlock the vehicle and confirm that both puddle lights illuminate properly.
